How about local perfusion? The treatment of localized cutaneous and lymphatic metastases by isolated hyperthermic limb perfusion using a combination of chemotherapeutic and immunotherapeutic agents has generated renewed interest following a successful European trial. In this treatment, the circulation of the affected limb is isolated, the blood slowly heated, and then it is returned to the limb along with high doses of melphalan with or without tumor necrosis factor or gammainterferon. Because the circulation of the limb is isolated from the remaining systemic circulation, much higher doses of the therapeutic agents can be administered than what could be tolerated systemically.
