Can a biopsy help in the differential diagnosis of patchy nonscarring alopecia? Yes. A biopsy would reveal nonscarring alopecia with hemorrhage, and pigmented follicular casts in trichotillomania. Alopecia areata shows a perifollicular lymphocytic infiltrate and small anagen hairs. Tinea capitis shows fungal elements within hair follicles. |
